I received this document from someone who wishes to remain anonymous. I have no idea if this document would help to avoid mandatory vaccination, but it's looks very interesting. The document is attached and the e-mail text is as follows:

For those of you who are concerned about the possibility of a mandatory/forced vaccination against the H1N1 Swine Flu virus, here is your way out.

The attached MS Word .doc file is a legal document that you can submit to the attending physician or nurse before he/she injects the poisonous cocktail into your bloodstream. DO NOT allow anyone to give you a swine flu vaccination (or any other vaccination for that matter) without signing the attached document. I can't imagine any doctor or nurse who would sign the Physician’s Warranty of Vaccine Safety as they are assuming responsibility for the risks attached to injecting these dangerous chemicals, tissues, and compounds into a human body.

If, for some reason, you decide to get the vaccination AND the doctor does sign the document, you have virtually won the lottery. When you begin to experience the myriad of symptoms and side affects that accompany these toxins, just call any attorney who will be happy to help you with the lawsuit. Remember that the federal government and the pharmaceutical companies are exempt from liability regarding the flu vaccines, but the administering physician is not exempt if you have them sign the document.
